Confidentiality & supervision
This disclosure applies to therapy services provided by Maureen Galindo, Licensed Marriage and Family Therapist Associate, supervised by Laurie Hightower, PhD. Therapy records are maintained in SimplePractice. Clinical cases may be discussed with Laurie Hightower for professional supervision. This practice provides services on a self-pay basis.
Patient communications and mental health records are subject to applicable Texas confidentiality law. Disclosure requires valid authorization or an applicable legal exception. Exceptions may include mandatory abuse or neglect reporting, qualifying threats of injury, authorized oversight, and judicial proceedings that satisfy applicable legal requirements. A subpoena or informal request does not, by itself, eliminate confidentiality protections.
The Self-Trust App is a separate educational service governed by its own Privacy & Use terms. It is not a confidential therapy portal. Public website forms and ordinary email must not be used to transmit clinical records or sensitive treatment information. Use the SimplePractice client portal for confidential administrative communication. Electronic communication and telehealth carry privacy risks; protect account credentials and use a private device and setting.
Your rights & records requests
Subject to applicable law, clients may request access to eligible treatment records, correction of inaccurate or incomplete information, confidential communications, restrictions on certain uses or disclosures, and an accounting of disclosures where available. Access to separately maintained psychotherapy notes and certain other information may be restricted. Requests made by an authorized representative require verification of that representative’s authority.
Request records or submit a privacy inquiry, or send a request through the SimplePractice client portal. Ordinary email should contain only a general administrative request, without clinical details or attachments. Identity verification and any required written authorization must be completed before release. Requests are subject to applicable response deadlines, permissible fees, and legally recognized limitations; any required explanation of denial will be provided. Records from treatment through another organization must be requested from that organization’s records custodian.
Any authorization to disclose information may generally be revoked in writing, except to the extent action has already been taken in reliance on it. This disclosure does not replace the practice-specific Notice of Privacy Practices and informed-consent documents applicable to treatment.
Privacy concerns & complaints
Privacy inquiries and concerns may be submitted to Maureen Galindo through the request link above or the SimplePractice client portal. Clients may also contact the appropriate regulatory authority directly. Filing a complaint does not require prior resolution with the provider and must not result in retaliation.
The Texas Behavioral Health Executive Council regulates marriage and family therapists and investigates professional misconduct. Not every disagreement involves professional misconduct; the Council can provide information about filing a complaint.

Good Faith Estimates
If you do not have insurance or are not using it to pay for care, you have the right to receive a written Good Faith Estimate of expected charges. You can request an estimate before scheduling. When you schedule at least three business days in advance, your provider generally must give you an estimate before care begins. Keep a copy.
If a provider’s bill is at least $400 above that provider’s estimate, you may qualify for the federal patient-provider dispute process. You generally must start that process within 120 calendar days of receiving the initial bill. Learn about estimates and billing disputes or call 1-800-985-3059.
